  • For those who are ready to “Fast Track” into the TLCEdge Surgical Weight Loss Program, you may complete the educational portion of the TLCEdge Program (normally covered in a seminar) in a consultation scheduled on the same day as your appointment for pre-operative assessment. These special "double up" sessions are reserved for those who wish to expedite the process. Please call 972-792-2799 or 877-TLC-EDGE Toll Free if you are ready to get started toward a new, healthier you. These special "double up" sessions are available only by calling a Care Coordinator.


  • A packet including patient forms will be provided at the free informational seminars or consultations. Or you may print the New Patient Forms at home by clicking here. These forms should be completed as thoroughly as possible prior to your pre-operative assessment appointment. You do not need to download the insurance related forms until instructed to do so by your Care Coordinator.
